1. PURPOSE
This Agreement outlines the terms under which Bobby Dodd Institute (BDI) will provide employment support, training, case management, and related community-based services to the Client. BDI’s mission is to empower people with differing abilities to maximize their self-sufficiency and employment opportunities.
2. SCOPE OF SERVICES
The Provider shall offer services based on the Client’s individualized service plan (ISP), which may include:
- Employment Services: Job readiness training, job coaching, supported employment, and placement.
- Training & Development: Skills development, resume preparation, workplace communication, and self-advocacy training.
- Community Integration: Support for participation in community-based activities and resources.
- Case Management: Ongoing assessment of progress, needs, and service coordination.
- Other Services: As agreed upon in writing and consistent with BDI’s mission and available programs.
3. ASSESSMENTS
Provider will conduct regular assessments of the Client’s progress toward goals. Frequency will be based on the Client’s ISP, typically on a monthly or quarterly basis.
4. TERM OF AGREEMENT
This Agreement shall commence on the date of signing and remain in effect for one (1) year, unless earlier terminated under Section 8. It may be renewed annually upon mutual agreement.
5. PAYMENT AND FUNDING
Services may be funded through one or more of the following:
- Medicaid waivers
- Vocational Rehabilitation (VR) programs
- Private pay
- Other state or community funding sources
Any costs not covered by funding agencies remain the responsibility of the Client/Guardian.
6. CONFIDENTIALITY & COMPLIANCE
Provider will maintain confidentiality of Client information in compliance with HIPAA and applicable federal and state privacy laws. Records will be kept securely and released only with Client/Guardian consent or as required by law.
7. LIABILITY
The Provider is not liable for outcomes outside the scope of its services or caused by factors beyond its control (e.g., employer practices, Client non-compliance). The Client/Guardian agrees to indemnify and hold harmless the Provider against such claims.
8. TERMINATION
Either party may terminate this Agreement with thirty (30) days’ written notice. Provider may terminate immediately in cases of:
- Unsafe or inappropriate behavior by Client or family toward staff or others
- Non-compliance with agreed-upon service plan or policies
- Funding denial or exhaustion
9. GOVERNING LAW
This Agreement shall be governed by the laws of the State of Georgia.
