LICENSING

ASSESSMENTS

As Licensed Providers, Seek2Find Home Care Services is required by the State of Georgia to have a Registered Nurse assess patients based on their condition. The initial assessment will be covered by the agency; subsequent assessments are the responsibility of the client.

Assessments will result in a $100 invoice increase per occurrence.

DESCRIPTION OF SERVICES

Seek2Find Home Care Services LLC shall provide, as applicable:

• Companionship Services
• Personal Care Assistance
• Transitional Care
• Chronic Disease Care
• Respite Care
• Surgery Assistance
• End-of-Life Support Care
• Private Duty
• Hospice Care
• Certified Nursing Assistant Services

Services include medical support coordination, personal hygiene assistance, nutrition, ambulation, transportation, and related home care duties.

PAYMENT

The client shall pay $_____ per hour at a monthly rate of $_____. Payments are due weekly to Seek2Find Home Care Services LLC via check, online banking, Medicaid, or approved insurance. Holidays will be billed at time-and-a-half. Mileage reimbursement will apply if the caregiver uses their vehicle for transport. Late payments incur a 3% fee.

NON-COMPETE

Clients may not personally hire Seek2Find employees or contractors during the term of this contract. Violation will result in full payment of contract value and legal fees.

INSURANCE

Seek2Find Home Care Services LLC carries General Liability and Employment Practices Liability Insurance with appropriate coverage levels.

CONFIDENTIALITY

All client and family information obtained during services is strictly confidential and may not be disclosed. This clause survives termination of the contract.

CANCELLATION

If services are canceled after signing, a 25% fee of the monthly agreed service amount will be charged.

FORCE MAJEURE

Neither party shall be held liable for delays or failure to perform caused by acts beyond reasonable control, including natural disasters, government sanctions, or power failures.

SEVERABILITY

If any provision of this Contract is deemed invalid, the remaining provisions shall remain enforceable to the fullest extent permitted by law.

AMENDMENT

This Contract may only be amended in writing, signed by all parties.

NOTICE

Notices shall be provided in person or by certified mail to the addresses listed above.

ATTORNEY'S FEES

The breaching party shall be responsible for attorney's fees and costs incurred by the other party in enforcement of this Contract.

APPLICABLE LAW

This Contract shall be governed by the laws of the State of Georgia.

SIGNATURES

This contract shall be signed and dated by the Client, Power of Attorney, or Guardian, and by Dr. Crystal Edwards, Owner and CEO of Seek2Find Home Care Services LLC.
