It is with great pleasure that Seek2Find Home Care Services LLC offers you the position of Personal Care Assistant. You will be reporting to Dr. Crystal Edwards, and your start date will be scheduled upon your completion of all required compliance documents and reference checks.

This is a PRN position [exempt position that is not eligible for overtime/nonexempt position eligible for overtime pay after 40 hours in a workweek] or full-time or part-time position. We are offering you a starting base wage of $17.00 OR $18.00 per hour paid biweekly. Please mark the pay agreed upon during the interview. The case location will be in Lawrenceville, Georgia.

This job offer is contingent upon the following:

• Completion of a satisfactory background check and fingerprinting.
• Satisfactory reference checks.
• Execution of an employment/noncompete/confidentiality agreement.
• Obtaining a current TB test.
• Proof of current Certified Nursing Assistant certification/licensure.

[Once the above contingencies are successfully completed, this job offer will also be contingent upon receipt of results of a negative Covid-19 test or proof of Covid-19 vaccination before reporting to any case.]

On your first day, you will be given an orientation. This orientation will include completing employment forms, introduction to management, receipt of the Seek2Find Home Care Services Employee Handbook, and training for your specific case. Please bring appropriate documentation for the completion of your new-hire forms, including proof that you are presently eligible to work in the United States for I-9 Form purposes. Failure to provide appropriate documentation within three days of hire will result in immediate termination of employment in accordance with the terms of the Immigration Reform and Control Act.
