Position: Registered Nurse or Licensed Practical Nurse _______________________

Position Summary

The Registered Nurse is responsible for ethical and effective patient care delivery. The Registered Nurse follows the Quality Improvement Program and adheres to State and Federal regulations in compliance with agency policies and procedures. The Registered Nurse also follows and maintains compliance with each patient's Plan of Care (POC).

Position Qualifications

1. Must be a licensed Registered Nurse in the State of Georgia.
2. The RN must have a background in public health, home care, or any hospital setting.
3. Must have at least 5 years of experience.
4. PRN position is as needed.
5. Competitive base pay.

Physical Qualifications

1. Must have excellent vocabulary skills.
2. Must be able to travel to patients' homes.
3. Must be able to physically stoop, lean, bend, lift, or transfer patients.

Duties and Responsibilities

Ability to lead and provide direction to prospective members of the patient care team through case management. The Nurse will demonstrate effective case management on a regular basis to reevaluate the needs of each client.

1. Coordinate intake and referral forms from an acceptable medical provider.
2. Complete initial in-home evaluations and/or assessments.
3. Establish each client's care plan/treatment plan.
4. Supervise the planning of the Plan of Care (POC) and ensure quality, ethical, and effective patient care is provided.
5. Follow up with other agencies, staff, and contracted services to coordinate and plan the POC.
6. Supervise Personal Care and Skilled Nursing Care services at least every 62 to 122 days, depending on the care plan and type of services provided. Documentation must be kept current in client records.
7. Supervise each CNA or PCA assigned to a caseload and evaluate patient plans and their effectiveness.
8. Perform other duties as assigned.
9. Actively participate in the agency’s quality improvement activities.
10. Provide client and staff supervision while on call.
11. Complete office paperwork and maintain timely charting of client and personnel information.
12. Participate in educational classes related to patient care services provided by the agency.
13. Develop and attend professional seminars to enhance agency needs and develop new strategies.
